CBD for MCAS: What Research Says About Mast Cells and Histamine

If you've searched for CBD for MCAS, CBD and histamine, or ways to support a healthy inflammatory response, you've probably encountered some big claims.

The science is more nuanced—and more interesting.

Emerging research suggests cannabidiol (CBD) interacts with immune and inflammatory pathways that may be relevant to mast cells and histamine responses. In a 2024 laboratory and animal study, researchers found that CBD suppressed antigen-stimulated mast cell degranulation in human-derived and mouse mast cells. But research specifically connecting CBD and mast cell activation syndrome (MCAS) is still limited, and CBD has not been established as a treatment for MCAS.

Here's what we know so far.

What Are MCAS, Mast Cells and Histamine?

Mast cells are immune cells found throughout the body. When activated, they release chemical mediators, including histamine, that play important roles in normal immune and allergic responses.

Mast cell activation syndrome, or MCAS, is a medical condition involving inappropriate mast cell activation and mediator release.

You may also hear MCAS discussed alongside histamine intolerance, but they aren't the same condition. Because symptoms can overlap and both can be complex, diagnosis and treatment should involve a qualified healthcare professional.

Can CBD Help With MCAS?

At this point, there isn't enough clinical evidence to say that CBD can treat MCAS.

What researchers are discovering is that CBD interacts with a surprisingly broad range of biological systems involved in immune and inflammatory responses.

A 2024 study investigated CBD's effects on mast cells specifically. Researchers found that CBD suppressed antigen-stimulated mast cell degranulation in laboratory-grown human and mouse mast cells. CBD also reduced an allergic response in a mouse model.

Interestingly, the researchers found that this effect didn't appear to depend on the familiar CB1 or CB2 cannabinoid receptors.

That's compelling mechanistic evidence—but it's not the same thing as demonstrating that CBD improves MCAS symptoms in people.

Does CBD Stabilize Mast Cells?

You'll sometimes see CBD described online as a "mast cell stabilizer."

The research doesn't yet justify making that statement broadly.

The 2024 study provides evidence that CBD can inhibit mast cell degranulation under certain experimental conditions.

But an earlier laboratory study produced a very different result: CBD increased beta-hexosaminidase release, a marker of mast cell activation, in a rat basophilic leukemia mast-cell model.

Reviews of the evidence therefore describe CBD's effects on mast cells as limited and sometimes inconsistent.

In other words: CBD and mast cells are an intriguing area of research, not a settled one.

Does CBD Reduce Histamine?

Histamine is one of the mediators released when certain mast cells degranulate. That makes the research into CBD and mast cell degranulation potentially relevant to histamine responses.

But there's an important distinction.

Evidence that CBD affects mast-cell activity in a laboratory doesn't establish that taking CBD will lower histamine levels or improve histamine intolerance in humans.

More human research is needed to understand that relationship.

CBD and Inflammatory Signaling

This is an area where the broader CBD research becomes particularly interesting.

CBD interacts with numerous components of the immune system and has been studied for its effects on cytokines and inflammatory signaling pathways.

Researchers have examined its interaction with targets and pathways including PPARγ, NF-κB, TRPV1 and the endocannabinoid system.

A comprehensive review of CBD and innate immunity found that CBD can influence multiple types of immune cells and inflammatory processes—but also emphasized that its effects can vary depending on the biological environment.

That complexity is one reason CBD continues to generate so much scientific interest.

What About CBD and the Gut?

Mast cells, the immune system and the gastrointestinal tract are closely connected.

In an animal study of intestinal inflammation, CBD reduced several signs of inflammation as well as mast-cell-associated mediators in intestinal tissue.

That's another potential biological connection researchers are exploring, but again, these findings came from an animal model and shouldn't be interpreted as evidence that CBD treats gastrointestinal symptoms associated with MCAS.

What Does Human Research Tell Us?

This is where the evidence gets much thinner.

There has been some human research involving related mast-cell disorders. For example, a small observational pilot study investigated pharmaceutical-grade CBD for pain in 44 people with mastocytosis and reported decreased pain scores after three months.

However, mastocytosis is not MCAS, the study focused on pain rather than histamine or mast-cell stabilization, and it wasn't a randomized placebo-controlled trial.

More recently, a 2025 case report described improvement in severe MCAS-associated pain in one patient treated with CBD oil in combination with another medication. A single case report can't establish effectiveness, but it demonstrates why researchers are interested in studying CBD and mast-cell disorders further.

CBD and MCAS: The Bottom Line

So, can CBD help with MCAS?

We don't know yet.

There's emerging evidence that CBD can interact with mast cells, immune signaling and inflammatory pathways. Some newer preclinical findings are particularly interesting.

But there isn't currently enough high-quality human clinical research to conclude that CBD treats MCAS, lowers histamine or reliably stabilizes mast cells in people.

That's not a reason to dismiss the research. It's a reason to follow it.
